Overall aim of cluster cooperation: The long-term goal of this cluster cooperation is to transform the health care landscape at the interface of countering the challenges of demographic change and capitalizing innovation at a NWE and wider EU level. Therefore. the cluster aims. within the timescale of funding. to set up the conditions which will allow for the widespread application of integrated. innovative and transferable health care models and solutions for healthy ageing and independent living. Specific cluster objectives: The cluster specific objectives reflect upon the horizontal issues of the project. i.e. (i) policy / regulatory framework and ethical issues. (ii) funding. (iii) cooperation and (iv) promotion of a transnational healthcare market (SMEs for innovation). ? PILLAR 1: EMPOWERMENT: Awareness raising and education for patients and health workforce. personal health management This pillar covers issues related to prevention. early diagnosis. health consciousness. self-care and participation of older people. It also includes raising awareness of the health workforce with regards to the latest healthcare related technologies and solutions. ? PILLAR 2: ORGANISATION: Innovative care models. management and organisational structures and cooperation This pillar relates to the efficient organisation and management of healthcare provision for older people. and also to boosting cooperation between the various actors of healthcare provision to enable the widespread deployment of innovative care models. ? PILLAR 3: TECHNOLOGIES: Technologies and solutions for independent living E.g. The project I-stay@home was in charge of the inventory and the results can be found by clicking here. I-stay@home compared the living labs based on different topics such as themes, types and stages. The living labs are located all over Europe as can be seen on the map in the link.The aims, focus and activities of the different living labs are thoroughly described in the files attached to the living labs in the database.
