Dementia is an umbrella term describing a variety of diseases and conditions as a result of brain cells damage, degeneration or death. Dementia is associated with changes in memory, behavior and reasoning and Alzheimer, the most common type of it. It is characterized by a progressive cognitive decline accompanied by functional impairments which have a direct impact on patient’s ability to perform activities of daily living. An active disease management can significantly improve quality of life through all stages of the disease for individuals with dementia and their caregivers. The ALZCARE project aims to assist physicians, individuals affected by dementia and their caregivers in the cross-border regions of Greece and Albania in the management of the disease as well as to offer supportive services in the form of tracking unattended patients outside their homes through the use of advanced ICT means. The added value of the system is that it offers the ability to register and analyze the current situation of dementia in the cross-border regions, provides a unified management system for assisting even the less experience physicians and providing supportive services (e.g. counseling and tracking of unattended patients).
